Jsme soukrom\u00fdm za\u0159\u00edzen\u00edm a zab\u00fdv\u00e1me ambulantn\u00ed specializovanou p\u00e9\u010di v oboru v\u0161eobecn\u00e9 praktick\u00e9 l\u00e9ka\u0159stv\u00ed (praktick\u00fd l\u00e9ka\u0159 pro dosp\u011bl\u00e9). N\u00e1\u0161 t\u00fdm je p\u0159ipraven V\u00e1m poskytnout \u0161pi\u010dkovou zdravotn\u00ed p\u00e9\u010di. Pomoci v tom m\u016f\u017eete i Vy sv\u00fdm odpov\u011bdn\u00fdm p\u0159\u00edstupem k l\u00e9\u010db\u011b a dodr\u017eov\u00e1n\u00ed l\u00e9ka\u0159em a o\u0161et\u0159uj\u00edc\u00edm person\u00e1lem stanoven\u00fdch pravidel a doporu\u010den\u00ed.&nbsp;<\/span><\/p>\n<p dir=\"ltr\" style=\"line-height: 1.295; margin-top: 0pt; margin-bottom: 8pt;\">\n<\/p><ul>\n<li aria-level=\"1\"><b>Identifikace<\/b><span style=\"font-weight: 400;\">&nbsp;<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">P\u0159i p\u0159\u00edchodu do ambulance neklepejte. Po p\u0159\u00edchodu sestry do \u010dek\u00e1rny p\u0159edlo\u017ete pr\u016fkaz zdravotn\u00ed poji\u0161\u0165ovny nebo \u017e\u00e1dost o proveden\u00ed l\u00e9ka\u0159sk\u00e9ho vy\u0161et\u0159en\u00ed. M\u016f\u017eeme po\u017eadovat, abyste p\u0159edlo\u017eili i pr\u016fkaz toto\u017enosti (ob\u010dansk\u00fd pr\u016fkaz, cestovn\u00ed pas). V p\u0159\u00edpad\u011b nep\u0159edlo\u017een\u00ed m\u00e1me pr\u00e1vo poskytnut\u00ed zdravotn\u00ed p\u00e9\u010de odm\u00edtnout. Hlaste n\u00e1m pros\u00edm zm\u011bnu poji\u0161\u0165ovny, jm\u00e9na \u010di p\u0159\u00edjmen\u00ed, bydli\u0161t\u011b \u010di kontaktn\u00edch \u00fadaj\u016f.&nbsp;<\/span><\/p>\n<p dir=\"ltr\" style=\"line-height: 1.295; margin-top: 0pt; margin-bottom: 8pt;\">\n<\/p><ul>\n<li aria-level=\"1\"><b>Ordina\u010dn\u00ed doba<\/b><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Ordina\u010dn\u00ed doba je zve\u0159ejn\u011bna na vstupn\u00edch dve\u0159\u00edch do zdravotnick\u00e9ho za\u0159\u00edzen\u00ed a na webov\u00fdch str\u00e1nk\u00e1ch <\/span><a href=\"https:\/\/www.snowmedica.cz\/\"><span style=\"font-weight: 400;\">https:\/\/www.snowmedica.cz\/<\/span><\/a><span style=\"font-weight: 400;\">.&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Ordina\u010dn\u00ed doba je rozd\u011blena na hodiny pro nemocn\u00e9=akutn\u00ed pacienty a zdrav\u00e9 pacienty (preventivn\u00ed prohl\u00eddky, o\u010dkov\u00e1n\u00ed, administrativn\u00ed \u00fakony, vy\u0161et\u0159en\u00ed k&nbsp;posudk\u016fm).&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">V ordina\u010dn\u00ed dob\u011b pro zdrav\u00e9 pacienty nen\u00ed mo\u017en\u00e9 o\u0161et\u0159en\u00ed nemocn\u00e9ho pacienta ani pacienta s&nbsp;d\u00e9letrvaj\u00edc\u00edmi obt\u00ed\u017eemi bez p\u0159edchoz\u00ed telefonick\u00e9 nebo e-mailov\u00e9 domluvy se sestrou \u010di l\u00e9ka\u0159em. Posledn\u00ed pacient je o\u0161et\u0159en 30 minut p\u0159ed koncem ordina\u010dn\u00ed doby.<\/span><\/p>\n<ul>\n<li aria-level=\"1\"><b>Objedn\u00e1v\u00e1n\u00ed do ordinace, po\u0159ad\u00ed pacient\u016f<\/b><\/li>\n<\/ul>\n<p dir=\"ltr\" style=\"line-height: 1.295; margin-top: 0pt; margin-bottom: 8pt;\">\n<\/p><p><span style=\"font-weight: 400;\">Na ve\u0161ker\u00e9 prohl\u00eddky, v\u010detn\u011b prohl\u00eddek pacient\u016f s&nbsp;d\u00e9letrvaj\u00edc\u00edmi neakutn\u00edmi obt\u00ed\u017eemi, na o\u010dkov\u00e1n\u00ed,&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">na potvrzen\u00ed, l\u00e9ka\u0159sk\u00e9 posudky \u010di jinou administrativu je nutn\u00e9 se p\u0159edem objednat. \u010cas, na kter\u00fd je pacient objedn\u00e1n, nep\u0159edstavuje \u010das, kdy pacient vstupuje do ordinace, ale \u010das, odkdy je pacient p\u0159\u00edtomen v&nbsp;\u010dek\u00e1rn\u011b. Dodr\u017eujte datum a \u010das objedn\u00e1n\u00ed, pokud z n\u011bjak\u00e9ho d\u016fvodu nebudete moci ve sjednan\u00e9m term\u00ednu dorazit, tak n\u00e1s pros\u00edm v\u010das informujte. Po\u0159ad\u00ed o\u0161et\u0159en\u00ed \u010di vy\u0161et\u0159en\u00ed pacient\u016f ur\u010duje zdravotnick\u00fd person\u00e1l. Vzhledem k&nbsp;charakteru a chodu ordinace v\u0161eobecn\u00e9ho praktick\u00e9ho l\u00e9ka\u0159e m\u016f\u017ee doj\u00edt k&nbsp;posunut\u00ed \u010dasu objedn\u00e1n\u00ed n\u011bkdy i o del\u0161\u00ed \u010dasov\u00fd interval z&nbsp;r\u016fzn\u00fdch p\u0159\u00ed\u010din. \u017d\u00e1d\u00e1me v\u0161echny \u010dekaj\u00edc\u00ed pacienty o pochopen\u00ed a toleranci.<\/span><\/p>\n<p dir=\"ltr\" style=\"line-height: 1.295; margin-top: 0pt; margin-bottom: 8pt;\">\n<\/p>\n<ul>\n<li aria-level=\"1\"><b>Pr\u016fb\u011bh vy\u0161et\u0159en\u00ed&nbsp;<\/b><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">U \u017e\u00e1dn\u00e9 zdravotn\u00ed slu\u017eby poskytovan\u00e9 zdravotnick\u00fdm za\u0159\u00edzen\u00edm nevy\u017eadujeme p\u00edsemn\u00fd souhlas pacienta vyjma p\u0159\u00edpad\u016f, kter\u00e9 v\u00fdslovn\u011b stanov\u00ed z\u00e1kon. Doporu\u010den\u00ed na specializovan\u00e1 vy\u0161et\u0159en\u00ed a doklad o do\u010dasn\u00e9 pracovn\u00ed neschopnosti (d\u00e1le jen DPN) vystavuje l\u00e9ka\u0159 na z\u00e1klad\u011b zji\u0161t\u011bn\u00ed zdravotn\u00edho stavu pacienta, nikoliv na \u017e\u00e1dost pacienta. DPN m\u00e1 v\u017edy stanoven\u00fd term\u00edn dal\u0161\u00ed kontroly nebo p\u0159edpokl\u00e1dan\u00fd term\u00edn ukon\u010den\u00ed DPN. Pokud se pacient bez p\u0159edchoz\u00ed omluvy nedostav\u00ed na pl\u00e1novanou kontrolu, bude mu DPN v&nbsp;dan\u00e9m term\u00ednu ukon\u010dena. Origin\u00e1ly zpr\u00e1v z&nbsp;odborn\u00fdch vy\u0161et\u0159en\u00ed je pacient povinen p\u0159in\u00e9st do ordinace osobn\u011b. \u017d\u00e1d\u00e1me pacienty aby nepos\u00edlali oskenovan\u00e9 zpr\u00e1vy, fotokopie a fotografie zpr\u00e1v e-mailem.<\/span><\/p>\n<p dir=\"ltr\" style=\"line-height: 1.295; margin-top: 0pt; margin-bottom: 8pt;\">\n<\/p><ul>\n<li aria-level=\"1\"><b>Zdravotnick\u00e1 dokumentace zpr\u00e1vy z&nbsp;odborn\u00fdch vy\u0161et\u0159en\u00ed&nbsp;<\/b><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Zdravotnick\u00e1 dokumentace pacienta veden\u00e1 l\u00e9ka\u0159em je majetkem l\u00e9ka\u0159e. Do dokumentace pat\u0159\u00ed origin\u00e1ly zpr\u00e1v od specialist\u016f, zpr\u00e1vy z odborn\u00fdch vy\u0161et\u0159en\u00ed, propou\u0161t\u011bc\u00ed zpr\u00e1vy atd. Nahl\u00ed\u017eet do dokumentace a po\u0159izovat z&nbsp;n\u00ed v\u00fdpisy a kopie m\u016f\u017ee pouze pacient \u010di jeho z\u00e1konn\u00ed z\u00e1stupci. Kopie si mus\u00ed pacient po\u0159\u00eddit p\u0159\u00edmo v&nbsp;ordinaci za \u00faplatu dle platn\u00e9ho cen\u00edku. Zpr\u00e1vy nelze pos\u00edlat e-mailem.<\/span><\/p>\n<p dir=\"ltr\" style=\"line-height: 1.295; margin-top: 0pt; margin-bottom: 8pt;\">\n<\/p><ul>\n<li aria-level=\"1\"><b>Cenn\u00e9 p\u0159edm\u011bty, pen\u00edze<\/b><span style=\"font-weight: 400;\">&nbsp;<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Doporu\u010dujeme do ambulance nevn\u00e1\u0161et v\u011bt\u0161\u00ed finan\u010dn\u00ed obnos, \u0161perky, \u010di cenn\u00e9 a drah\u00e9 p\u0159edm\u011bty. Na\u0161e ambulance neponese \u017e\u00e1dnou odpov\u011bdnost v p\u0159\u00edpad\u011b jejich ztr\u00e1ty \u010di odcizen\u00ed. Nenech\u00e1vejte \u017e\u00e1dn\u00e9 sv\u00e9 v\u011bci v \u010dek\u00e1rn\u011b, pokud v n\u00ed nejste sami p\u0159\u00edtomni. Prostory ordinace jsou vybaveny v\u011b\u0161\u00e1ky na kab\u00e1ty, kde si sv\u00e9 osobn\u00ed v\u011bci m\u016f\u017eete pohodln\u011b odlo\u017eit po dobu vy\u0161et\u0159en\u00ed\/o\u0161et\u0159en\u00ed u n\u00e1s. Osobn\u00ed v\u011bci nenech\u00e1vejte bez dozoru.&nbsp;<\/span><\/p>\n<p dir=\"ltr\" style=\"line-height: 1.295; margin-top: 0pt; margin-bottom: 8pt;\">\n<\/p><ul>\n<li aria-level=\"1\"><b>Vnit\u0159n\u00ed re\u017eim<\/b><span style=\"font-weight: 400;\">&nbsp;<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Pro zv\u00fd\u0161en\u00ed bezpe\u010dnosti pacient\u016f i person\u00e1lu je do na\u0161\u00ed ambulance zak\u00e1z\u00e1no vn\u00e1\u0161et a ukl\u00e1dat zde jak\u00e9koliv zbran\u011b, st\u0159elivo a nebezpe\u010dn\u00e9 l\u00e1tky. Pokud byste u sebe zbra\u0148 m\u011bl\/a, je bezpodm\u00edne\u010dn\u011b nutn\u00e9, abyste tuto skute\u010dnost ozn\u00e1mil\/a o\u0161et\u0159uj\u00edc\u00edmu person\u00e1lu. P\u0159i pobytu v ambulanci plat\u00ed z\u00e1kaz po\u017e\u00edv\u00e1n\u00ed alkoholick\u00fdch n\u00e1poj\u016f a n\u00e1vykov\u00fdch l\u00e1tek. Ve v\u0161ech prostor\u00e1ch ambulance plat\u00ed z\u00e1kaz kou\u0159en\u00ed.&nbsp;&nbsp;<\/span><\/p>\n<p dir=\"ltr\" style=\"line-height: 1.295; margin-top: 0pt; margin-bottom: 8pt;\">\n<\/p><ul>\n<li aria-level=\"1\"><b>Doprovod pacienta&nbsp;<\/b><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">V p\u0159\u00edpad\u011b doprovodu pacienta je nutn\u00e9, aby V\u00e1\u0161 doprovod neru\u0161il provoz ambulance a pln\u011b se \u0159\u00eddil pokyny na\u0161ich pracovn\u00edk\u016f. Je zak\u00e1z\u00e1no ponech\u00e1vat v \u010dek\u00e1rn\u011b \u010di v jin\u00fdch prostor\u00e1ch ambulance d\u011bti bez dozoru. D\u011bti, pokud je to mo\u017en\u00e9, do \u010dek\u00e1rny \u010di ordinace nevo\u010fte, zbyte\u010dn\u011b je vystavujete riziku n\u00e1kazy.&nbsp;<\/span><\/p>\n<p dir=\"ltr\" style=\"line-height: 1.295; margin-top: 0pt; margin-bottom: 8pt;\">\n<\/p>\n<ul>\n<li aria-level=\"1\"><b>Odm\u00edtnut\u00ed p\u0159ijet\u00ed pacienta do p\u00e9\u010de&nbsp;<\/b><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">V souladu s ustanoven\u00edm \u00a7 48z\u00e1kona \u010d. 372\/2011 Sb. m\u016f\u017ee poskytovatel odm\u00edtnout p\u0159ijet\u00ed pacienta do p\u00e9\u010de pokud by vzd\u00e1lenost m\u00edsta pobytu neumo\u017e\u0148ovala v&nbsp;p\u0159\u00edpad\u011b poskytov\u00e1n\u00ed zdravotn\u00edch slu\u017eeb v&nbsp;oboru v\u0161eobecn\u00e9 praktick\u00e9 l\u00e9ka\u0159stv\u00ed v\u00fdkon n\u00e1v\u0161t\u011bvn\u00ed slu\u017eby nebo jeho p\u0159ijet\u00ed br\u00e1n\u00ed provozn\u00ed d\u016fvody.<\/span><\/p>\n<p dir=\"ltr\" style=\"line-height: 1.295; margin-top: 0pt; margin-bottom: 8pt;\">\n<\/p><ul>\n<li aria-level=\"1\"><span style=\"font-weight: 400;\">&nbsp;&nbsp;<\/span><b>\u00dahrada p\u00e9\u010de&nbsp;<\/b><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Nen\u00ed-li poskytovan\u00e1 zdravotn\u00ed p\u00e9\u010de pln\u011b hrazena z ve\u0159ejn\u00e9ho zdravotn\u00edho poji\u0161t\u011bn\u00ed ani proplacena ze smluvn\u00edho poji\u0161t\u011bn\u00ed pacienta, je pacient povinen tuto p\u00e9\u010di uhradit p\u0159\u00edmo na\u0161\u00ed ambulanci. Jedn\u00e1 se nap\u0159\u00edklad o \u010dinnost posudkovou. V\u00fdkony a slu\u017eby, kter\u00e9 jsou prov\u00e1d\u011bny na \u017e\u00e1dost pacienta a nejsou hrazeny z ve\u0159ejn\u00e9ho zdravotn\u00edho poji\u0161t\u011bn\u00ed, si hrad\u00ed pacient s\u00e1m dle cen uveden\u00fdch v cen\u00edku v\u00fdkon\u016f.<\/span><\/p>\n<ul>\n<li aria-level=\"1\"><b>&nbsp;Jedn\u00e1n\u00ed a chov\u00e1n\u00ed ke zdravotnick\u00fdm pracovn\u00edk\u016fm a dal\u0161\u00edm pacient\u016fm&nbsp;<\/b><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Nikoho verb\u00e1ln\u011b ani fyzicky nenapadejte ani s n\u00edm nejednejte hrub\u011b \u010di vulg\u00e1rn\u011b. We are a private institution providing outpatient specialized care in the field of general medical practice (general practitioner for adults). Our team is ready to provide you with top-quality healthcare. You can help us by taking a responsible approach to your treatment and following the rules and recommendations set by the doctor and the medical staff.<\/span><\/p><ul><li aria-level=\"1\"><b>Identification\u00a0<\/b><\/li><\/ul><p><span style=\"font-weight: 400;\">When entering the office, do not knock. Upon the nurse\u2019s arrival in the waiting room, present your health insurance card or request for a medical examination. We may also ask you to present an identity card (ID, passport). If you fail to present these documents, we have the right to refuse healthcare services. Please inform us of any changes in your insurance provider, name, surname, address, or contact details.<\/span><\/p><ul><li aria-level=\"1\"><b>Office Hours\u00a0<\/b><\/li><\/ul><p><span style=\"font-weight: 400;\">Office hours are posted on the entrance doors to the healthcare facility and on our website: <\/span><a href=\"https:\/\/www.snowmedica.cz\/\"><span style=\"font-weight: 400;\">https:\/\/www.snowmedica.cz\/<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><span style=\"font-weight: 400;\"><br \/><\/span><span style=\"font-weight: 400;\">The office hours are divided into slots for sick patients (acute patients) and healthy patients (preventive check-ups, vaccinations, administrative tasks, medical examinations for reports).<\/span><span style=\"font-weight: 400;\"><br \/><\/span><span style=\"font-weight: 400;\">During the office hours for healthy patients, we cannot treat sick patients or patients with long-term issues without prior phone or email arrangement with the nurse or doctor. The last patient will be treated 30 minutes before the end of office hours.<\/span><\/p><ul><li aria-level=\"1\"><b>Appointments and Patient Order\u00a0<\/b><\/li><\/ul><p><span style=\"font-weight: 400;\">All check-ups, including check-ups for patients with long-term non-acute issues, vaccinations, confirmations, medical reports, or other administrative matters must be scheduled in advance. The appointment time refers to when the patient is expected to be present in the waiting room, not when the patient enters the office. Please adhere to the scheduled date and time. If you cannot make the scheduled time, kindly inform us in advance. The order of treatment or examination of patients is determined by the medical staff. Due to the nature of a general practitioner\u2019s office, appointment times may be delayed for various reasons. We ask all waiting patients for understanding and tolerance.<\/span><\/p><ul><li aria-level=\"1\"><b>Examination Procedure\u00a0<\/b><\/li><\/ul><p><span style=\"font-weight: 400;\">For any medical services provided by our facility, we do not require written consent from the patient except for cases explicitly specified by law. Referrals for specialized examinations and certificates for temporary work incapacity (DPN) are issued by the doctor based on the patient&#8217;s health status, not upon patient request. A DPN always has a scheduled follow-up date or an expected end date. If the patient misses the scheduled follow-up without prior notice, the DPN will be terminated on the scheduled date. The patient is required to bring original reports from specialized examinations to the office personally. We ask patients not to send scanned reports, photocopies, or photos of reports via email.<\/span><\/p><ul><li aria-level=\"1\"><b>Medical Documentation and Specialist Reports\u00a0<\/b><\/li><\/ul><p><span style=\"font-weight: 400;\">The patient&#8217;s medical documentation, maintained by the doctor, is the property of the doctor. It includes original reports from specialists, specialist examination reports, discharge reports, etc. Only the patient or their legal representatives can access and request copies of the documentation. Copies must be obtained directly from the office for a fee according to the current price list. Reports cannot be sent by email.<\/span><\/p><ul><li aria-level=\"1\"><b>Valuables and Money\u00a0<\/b><\/li><\/ul><p><span style=\"font-weight: 400;\">We recommend not bringing large amounts of money, jewelry, or valuable items into the office. Our office is not responsible for the loss or theft of such items. Do not leave your belongings unattended in the waiting room unless you are the only one present. The office is equipped with coat racks where you can comfortably leave your personal belongings during your examination\/treatment. Please do not leave personal belongings unattended.<\/span><\/p><ul><li aria-level=\"1\"><b>Internal Rules\u00a0<\/b><\/li><\/ul><p><span style=\"font-weight: 400;\">For the safety of both patients and staff, it is forbidden to bring any weapons, ammunition, or dangerous substances into our office. If you are carrying a weapon, it is mandatory to inform the medical staff. The consumption of alcoholic beverages and narcotics is prohibited in the office. Smoking is also prohibited in all areas of the office.<\/span><\/p><ul><li aria-level=\"1\"><b>Accompaniment of Patients\u00a0<\/b><\/li><\/ul><p><span style=\"font-weight: 400;\">If you are accompanied by someone, please ensure that they do not disrupt the operation of the office and fully follow the instructions of our staff. Children should not be left unattended in the waiting room or other areas of the office. If possible, do not bring children into the waiting room or examination room, as this unnecessarily exposes them to the risk of infection.<\/span><\/p><ul><li aria-level=\"1\"><b>Refusal of Patient Care\u00a0<\/b><\/li><\/ul><p><span style=\"font-weight: 400;\">In accordance with \u00a7 48 of Act No. 372\/2011 Coll., the provider may refuse to accept a patient if the location of their residence makes it impossible to provide home visits in general medical practice or if operational reasons prevent their acceptance.<\/span><\/p><ul><li aria-level=\"1\"><b>Payment for Care\u00a0<\/b><\/li><\/ul><p><span style=\"font-weight: 400;\">If the healthcare services provided are not fully covered by public health insurance or reimbursed by the patient\u2019s contractual insurance, the patient is obligated to pay directly to our office. This applies, for example, to medical examinations.
