Entry examination

At least 2 days in advance, it is necessary to order for blood tests according to your job classification (category 2 or 3).

The examination must also be accompanied by an extract from the medical documentation from your registering general practitioner (for foreigners, a completed questionnaire confirmed by a general practitioner from the country of origin).

Exit examination

No later than 2 days in advance, it is necessary to order for blood tests according to your job classification (category 2 or 3) depending on the validity of the entry or periodical inspections (validity usually 4 years up to 50 years of age).

The exit inspection is not mandatory, it should be carried out at the request of the employee on the day of leaving the job.

Periodical examination

At least 2 days in advance, it is necessary to order for blood tests according to your job classification (category 2 or 3).

Irregular examination

It is carried out after incapacity for work (sick leave) lasting more than 8 weeks or in case of inability to work within the current job classification (e.g. pregnancy).

After incapacity for work, you must also submit a statement from the general practitioner or specialist with whom you were incapacitated.

Blood tests necessary in case of pregnancy, other cases only as agreed in the doctor’s office.

Blood tests for periodical examinations for registered patients are carried out every 2 years

Category 2 = work in a laboratory, risk of chemicals, work with biological agents

Category 3 = work in the laboratory – department of virology + work with viral hepatitis B, control of antibodies after vaccination against hepatitis B + work with lasers (necessary eye examination)

Arrive on an empty stomach, do not eat from midnight, drink a lot of water in the evening and morning before the blood tests, bring your morning urine (medium flow) with you exclusively in a tube intended for this purpose, which you can buy at the pharmacy or pick up directly at the doctor’s office.

It is NECESSARY to bring with you the Request for a PL care inspection confirmed by the personnel department.
